Environmental pollution, ecological changes, lack of balanced nutritious diet, increased traffic, continued mental and economic pressure, all contribute to the chance of becoming sick. It is true that almost everybody could afford outpatient treatment, but when hospitalization is required, the heavy expenditure incurred for hospital treatment adds to the worry, burden and anxiety. In order to be free from the worry, burden and anxiety of hospitalization treatment expenses you are requested to take a health insurance scheme of Prime Islami Life. By paying a small premium you can ensure & choose a high level of Hospitalization treatment expenses.

Who can take the Plan?
Any organization/institution having at least 20(Twenty) insurable members can join the plan. Insurable member includes - employee, their spouse and dependent children provided that at least 95% of its eligible employees/members aged between 18 and 60 years happily join the plan. A new employee can join the plan after 90 (ninety) days of continuous active service with the employer.

How the Employee Benefits
Individuals and families have a great need for health and life insurance plans. Unfortunately, many people are unable to purchase individual coverage because of preexisting medical- conditions and/or the high cost of purchasing insurance. Therefore, many people rely on employer-sponsored plans as their sole source of life and health insurance. An employer- sponsored plan provides the following benefits for employees:
• Virtually all members of a given group may be insured, regardless of their prior health history.
• If the plan is contributory, the employee usually pays a lower rate per unit of benefits than is available with individual coverage.

PILIL Hospitalization Plan is designed to cover the expenses incurred for hospital treatment which in broad terms includes:
• Hospital accommodation in private room
• Consultation with physicians and/or surgeons
• Medical investigations and tests
• Major and intermediate surgical operations
• Use of operation theatre, anesthesia and other services
• Medicines
• Ancillary services like Labour room, Post-operative care and Intensive care facility, Blood transfusion, Oxygen therapy etc.

How to take the Plan?
Simply each and every employee shall fill up a Member's Application Form detailing his or her heath status. The employer shall fill up Employer's Application Form and forward it along with Members Form to us for their coverage. No medical examination or attendance before a physician is required. However, Prime Islami Life reserves the right to ask for a medical examination if necessary. Prime Islami Life will then calculate and submit premium bill for payment. On receipt of premium, Prime Islami Life will issue 'Cover Note' taking risk of insurance and take further necessary steps for signing up an agreement with the Employer.
How to pay the Premium?
Premium is payable in advance before the date of commencement of risk. The Employer usually pays the whole premium. In case of contributory plan (i.e., the employee pays part of the cost of the plan), the Employer is required to collect and deposit the premiums and communicate with Prime Islami Life on behalf of the Employees.
PILIL Hospitalization and Claim procedure

Prior to admission in a hospital/clinic the patient (Employee) will simply complete a Claim Notification Form (available with the employer) and submit it to Employer along with consulting Doctor's Advice Note. The Employer will send it to Prime Islami Life to obtain a letter of authorization for admission and advise the patient accordingly.

The patient shall submit the claim through the Employer in the prescribed FORM along with the original supporting documents for reimbursement of expenses.

In case of emergency, prior authorization is not necessary but Prime Islami Life should be notified through the Employer within 48 hours of admission.
